Job Description

Registered Nurse for the older adult program provided general nursing and behavioral health support for older than 60 adult consumers. Acts as liaison between physical/medical health and mental/behavioral health care. Performs nursing assessments, administers injections, educates and assist consumer with medication management. Coordinate health care between psychiatrist, medical providers (PCP and Specialists).

EXAMPLES OF WORK PERFORMED:
  • May administer medications, immunizations, treatment and observes patients for unusual symptoms and reactions.
  • May conduct physical, developmental, and psychosocial assessments of consumers health needs and implement appropriate action.
  • Obtains lab specimens needed for routine labs.
  • Instruct, counsel, and assist consumers in meeting their health care needs.
  • May make appropriate referrals to physician office or community resources.
  • Identify community services and make referrals to health care services.
  • Assists in evaluating services provided and makes recommendations for improvements.
  • Participate in quality assurance reviews as required.
  • Participates in weekly collaborative care staffing to develop and contribute Health goals for individual Recovery Plans.
  • Perform related duties as assigned.
Experience:
At least 1-year psychiatric nursing experience.
Must have valid driver's license, liability insurance, and reliable transportation.
Education:
  • Graduation from an accredited four-year college or university with major coursework in nursing or from an accredited nursing program is required.
  • Current and unencumbered licensure as a registered nurse in the state of Texas.
  • Applicant must be able to obtain credentialing as a Qualified Mental Health Professional through training and competency evaluations within 30 days of employment to be able to maintain employment in this position.
KNOWLEDGE, SKILLS, AND ABILITIES
  • Must possess excellent oral and written communication skills and computer skills for inputting clinical documentation.
  • Must have excellent time management and organizational skills.
  • Must demonstrate a caring attitude and communicate effectively with consumers, other staff and family members.
  • Ability to work as a team member in all areas of planning and assist individuals in meeting their goals.
  • Must be able to make decisions based on sound judgement in collaboration with other team members.
  • Knowledge of nursing techniques and procedures, health care laws and regulations, medical diagnoses and procedures, accepted medical treatment programs, and community health and nursing care principles, practices, and procedures.
  • Ability to provide patient care, prepare and maintain records, explain public health law, and to train others.
  • Ability to learn about and implement trauma-informed care practices within the scope of work for the position. 
  • Must have and maintain a level of general health that permits the performance of the job duties and ability to maintain regular attendance as set forth in Center policies and procedures. Stooping, bending, kneeling, sitting, standing, and keyboarding for prolong periods is required.
  • Must be able to perform skills required by CPR, First Aid, and PMAB certification.
  • Visit with consumers in office, home, or community. Assessing safety, assessment of physical and behavioral health needs. Assessment of medication management/teaching and communicate with psychiatrist medication adherence, possible adverse reaction or side effects, communicate needs for refills if necessary.
  • Ability to be able to lift 50 lbs.

Hours: Mon-Fri 8AM-5PM

Salary: $33.00 Hourly; $68,640.00 Annually
