Service Areas:
Accokeek, Brandywine, Cheltenham, Clinton, District Heights, Ft. Washington, La Plata, Oxon Hill, Suitland, Temple Hills, Waldorf, White Plains
What You’ll Do
- Conduct client intake assessments and ongoing reassessments
- Develop, update, and monitor individualized care plans
- Oversee care management for both clients and caregivers
- Collaborate with an interdisciplinary team to enhance care practices
- Mentor and train caregivers, supporting their professional growth
- Lead and assist with Caregiver Skills Training Evolutions
- Promote client safety and respond promptly to concerns
- Maintain accurate, timely documentation for all client interactions
What We’re Looking For
- Active RN license in Maryland or a compact state
- At least 1 year of long-term care experience preferred (motivated new grads and retired RNs welcome!)
- Strong commitment to confidentiality, ethics, and client dignity
- Willingness to travel throughout our service area
- Excellent communication and organization skills
- Positive, collaborative attitude and the ability to work independently
